Insurance Coverage
Contact your Durable Medical Equipment provider (DME) who sends your monthly tube feeding supplies.
Let them know about the FreeArm and why it is medically necessary for your family.
Ask nicely if they will provide a FreeArm and bill your insurance with the following LMN and PWO documents, even if your DME is not on the list below.
The more families that advocate for their DME to provide the FreeArm, the more DMES will see the importance! You can use the LMN and PWO and work with your case manager, dietitian or doctor’s nurse to print the LMN on your doctors letterhead and ask your doctor to sign both forms.
The FreeArm is insurance billable under HCPCS codes B9998 or E0776+A9900 and reimbursement numbers: Green- 50010-018203, Pink- 50010-018204, White- 50010-018205 and Blue- 50010-018206 or can be billed under the per diem rate for inpatient hospital stays. Wholesale discounts are available.
